Dr. Aazami is a licensed US Civil Surgeon authorized by the Bureau of Citizenship and Immigration Services to conduct Immigration Physicals and Immigration Exams for the Department of Homeland Security.


The Cost of the Immigration Physical Examination depends on Vaccination Requirements and Includes the Following:
- Basic physical examination
- Required blood work
- QuantiFeron TB test
- Vaccination status verification
- Copies of your laboratory results
- Certified and sealed I-693 Form
- We have the required forms at the office
Documents to bring with you to the immigration exam or immigration physical:
- A valid, government-issued picture ID such as a driver's license or passport (preferred).
- Proof of prior vaccinations.


Immunization Requirements
You are required to bring all of the documents in your possession about your immunization status to your first appointment. The documented evidence of your immunizations will be transferred into your supplemental I-693 form. The USCIS Required Immunizations by Age table is your guidance for your vaccination needs. According to the table, individuals without proof of immunization are required to be vaccinated before their I-693 can be signed off. You have the option to receive the vaccination either at the County Health Department, your private physician’s office, or in our office at the time of your USCIS physical examination.

About the Medical Examination
- Only civil surgeons designated by the USCIS may perform medical examinations.
- The doctor must sign and endorse the form before it is returned to you.
- If you have performed X-rays, you should be provided with the films once cleared.
- Applicants that require follow-up sessions must include the supplement to Form I-693.
- Form I-693 must be completed in English. Form I-693 is delivered to you in a sealed envelope (DO NOT OPEN).
- The applicant is responsible for paying for all fees related to the medical examination.

- Syphilis for applicants 15 years or older
- HIV (blood test)
